These documents are not to be altered for any reason.

IMPORTANT NOTE: All Students, including visiting guests, must have a Parental Consent Form, a Student Information Form, and a Parent Involvent Form on file before participating in FPCJC Youth Events.  Please, understand that this is important in providing parents and guardians with greater peace of mind, a richer experience for both students and adults, and for issues of Church liability.
